PWDA is currently developing our new Disability and Inclusion Action Plan (DIAP) with the assistance of access consultant Morwenna Collett. This plan will help ensure that we are accessible and inclusive to everyone in our community, and are actively working toward a society in which the human rights, belonging, contribution, potential and diversity of all people with disability are recognised, respected and celebrated with pride.
We have released this short survey to identify the effectiveness of current access and inclusion practices and potential areas of improvement. We are keen to hear from PWDA members and stakeholders, including people with disability, partners, funders, community members and disability, health and aged care service providers
All responses are anonymous and we encourage you to be honest with your responses.
The survey closes 11:59pm AEDT on Sunday 12 November 2023.

There are 13 questions in this survey, which are a mix of multiple choice and free text responses. If you feel you don’t know enough to comment on a particular section, please answer as best you can or leave this section blank.

At the end of the survey, you will have the option to include your email address to go into the draw to win one of two $200 cash incentives for your participation. The winner will be selected at random after the survey closing date.
